Hazard descriptionA fire occurred on an example of the Ultranox burner head, highlighting a issue with the design of the burner seal, and giving rise to a risk of fire._x000D_ The product did not satisfy the requirements of the Pressure Equipment (Safety) Regulations 2016.
RemedyThe manufacturer has undertaken a review of the burner heads and undertaken a design upgrade. The following voluntary corrective action has also been taken: All units currently in production or on-site awaiting final commissioning will be fully checked and tested before they are entered into service. Checked the integrity of the head design and conducted combustion checks on all units already supplied and have determined that all of these are operating without issue. One unit is to have the combustion head replaced.
